Filing a notification of wrongdoing

Ústav živočišné fyziologie a genetiky AV ČR, v. v. i. se sídlem Rumburská 89, Liběchov, PSČ 277 21, IČO 67985904, tímto dle zákona č. 171/2023 Sb., o ochraně oznamovatelů, umožňuje podat oznámení o možném protiprávním jednání vztahujícím se k němu jakožto povinnému subjektu on-line prostřednictvím dále uvedeného oznamovacího formuláře. S obsahem oznámení bude seznámena pouze příslušná osoba.

Institute of Animal Physiology and Genetics CAS, with its registered office at Rumburská 89, Liběchov, ZIP code 277 21, ID number 67985904, Czech Republic, hereby, in accordance with Act No. 171/2023 Coll., on the protection of whistleblowers, allows you to submit a notification about a possible wrongdoing relating to its as a mandatory subject online via the following notification form. Only the relevant person will be informed of the content of the notification.

Oznámení by mělo obsahovat identifikaci a kontaktní údaj oznamovatele, aby mohla příslušná osoba oznámení posoudit a ve spolupráci s oznamovatelem případně doplnit.

The notification should contain the identification and contact information of the notifier, so that the relevant person can assess the notification and possibly supplement it in cooperation with the notifier.

Popis protiprávního jednání
Description of wrongdoing

Popis protiprávního jednání by měl vylíčit co nejvíce rozhodujících skutečností o oznamovatelem identifikovaném protiprávním jednání. Měl by obsahovat popis jednání, identifikaci osob, které takto jednaly, určení času, kdy k takovému jednání došlo a případně další informace, které oznamovatel považuje za důležité.

The description of the wrongdoing should describe as many decisive facts as possible about the wrongdoing identified by the whistleblower. It should contain a description of the action, the identification of the persons who acted in this way, the determination of the time when such action took place and possibly other information that the whistleblower considers important.

Poznámka či jiné sdělení

Oznámení lze doplnit poznámkou či jiným sdělením, které si oznamovatel přeje uvést příslušné osobě.

The notification can be supplemented with a note or other message that the notifier wishes to give to the relevant person.

Oznámení lze doplnit vedlejším sdělením, které si oznamovatel přeje sdělit příslušné osobě.
The notification can be supplemented with a secondary message that the notifier wishes to communicate to the relevant person.

Informace o zpracování osobních údajů
Information on the processing of personal data

Osobní údaje uvedené v oznámení bude zpracovávat příslušná osoba, případně Ústav živočišné fyziologie a genetiky AV ČR, v. v. i. (nikoliv o totožnosti oznamovatele), a to pro účely posouzení oznámení o možném protiprávním jednání, jeho doplnění či upřesnění, návrhu opatření k jeho odstranění či prevenci a ochraně před případnými odvetnými opatřeními. Zpracování se provádí na základě plnění povinností daných zákonem č. 171/2023 Sb., o ochraně oznamovatelů, a to po dobu stanovenou tímto zákonem.

The personal data mentioned in the notification will be processed by the relevant person, or the Institute of Animal Physiology and Genetics CAS (not about the identity of the notifier), for the purposes of assessing the notification of a possible wrongdoing, supplementing or clarifying it, proposing measures to its removal or prevention and protection against possible retaliatory measures. The processing is carried out on the basis of the fulfillment of the obligations set out in Act No. 171/2023 Coll., on the protection of whistleblowers, for the period specified by this Act.

V případě vyplnění formuláře prověďte prosím jeho odeslání prostřednictvím následujícího tlačítka. Oznámení bude odesláno a zpřístupněno pouze příslušné osobě, přičemž ta potvrdí jeho přijetí, pakliže byl uveden alespoň jeden kontaktní údaj oznamovatele.
